1. I, the undersigned, am the legal owner of the vehicle with VIN and do hereby authorize R&T Body Shop, LLC (hereinafter referred to as R&T) and its employees and agents to repair said vehicle pursuant to the repair estimate.

 

2. I understand that the repair estimate is based upon a preliminary inspection. Additional labor, subcontract services, parts, and materials may ultimately be required to complete the repair.

 

3. I authorize R&T and its employees and agents to drive the vehicle for purposes including, but not limited to, diagnosis, testing, and inspection.

 

4. I agree to remove all valuables and personal property from the vehicle and understand that R&T will not be held responsible for loss or theft of any personal property left in the vehicle.

 

5. I understand that R&T will not be responsible for any vehicle rental expenses incurred during the repair process.

6. I understand that my vehicle needs to be dropped off with no less than 1/4 tank of gas or there will be a $10 charge plus cost of gas if required to put 5gal of gas in vehicle to move it to a sublet location if vehicle requires other repairs that we are not able to provide.

 

7. I authorize my insurance company to pay R&T directly for any and all repairs including supplemental payments. I hereby appoint R&T as a true and lawful attorney in fact to endorse, on my behalf, any insurance check received as payment for repairs performed on the vehicle.

 

8. R&T accepts Insurance Check (best method), Debit Card, Credit Card, Check, and Cash. A transaction fee of 3.5% will be added to all debit and credit card payments over $1,000. I am responsible for payment in full for the repairs except to the extent an insurance payment has been made. I understand that the vehicle will not be released to me until payment has been made in full. WE DO NOT OFFER PAYMENT PLANS

** THIS MEANS YOU MUST PROVIDE THE INITIAL INSURANCE CHECK PAYEMNT AT TIME OF PICK UP, IF YOU DID NOT RECEIVE IT, IT IS YOUR RESPONSIBILTY TO FOLLOW UP WITH THE INSRUANCE COMPANY ABOUT PAYMENT.

THERE WILL BE A $35 FEE ON ALL BOUNCED CHECKS.

BAD CHECKS OVER $500 ARE SUBJECT TO FELONY CHARGES

9. I understand that the vehicle may be deemed abandoned if I fail to retrieve the vehicle within 30 days of completion of the repair for any reason including failing to make payment in full. In such event, R&T may, in its sole discretion, impound the vehicle with a third party or seek to declare the vehicle abandoned for purposes of resale. Any proceeds from such resale shall first go to pay costs then to the balance owed to R&T then to the owner.

 

10. I understand that R&T is not responsible for any damage to the vehicle resulting from any act of nature including, without limitation, damage caused by hail, wind, fire, or flood.

 

11. R&T provides a limited warranty for all repairs, paint and workmanship on sheet metal, frame, unitized chassis components, and mechanical items, but specifically excludes electric systems. This limited warranty is subject to any manufacturer’s warranty covering parts and materials. This limited warranty is non-transferable. During the limited warranty period, R&T will repair or replace any defects caused by R&T or its agents. All warranty repairs must be performed at R&T. Any repairs or alterations to the vehicle performed otherwise shall automatically void this limited warranty. This limited warranty shall not apply to repairs necessitated by any cause beyond the reasonable control of R&T, including any defects, damage, or malfunctions caused by or resulting from unauthorized service or parts, improper or inadequate vehicle maintenance, alterations, accidents, modification or repairs, subsequent repairs performed by a party other than R&T, abuse, misuse, neglect, acts of God, or environmental damage.

 

12. This Repair Authorization constitute the entire agreement and supersedes all prior oral and written agreements and understandings of the parties. No modification, amendment, supplement or waiver shall be binding upon either party unless made in writing and signed by both parties.
